This guidance refers to:

  • Adults aged over 18 living in the Kirklees district
  • Carer's living in the Kirklees district whether or not they are eligible to support from the council
  • Kirklees District Council Care Navigators

This guidance does not cover:

  • Young people under the age of 18
  • People living outside the Kirklees district


Referral Criteria

  • Referrals to the care navigation team can be made for adults and carers who need:
    • help to access the support identified in their support plan
    • help to maintain and maximise independence, choice and control regarding their support
    • support to access health and social care
    • help at an early stage to prevent the need for more urgent or longer term support.
    • information about local groups, organisations, suppliers and services
    • Information about direct payments, community support and voluntary organisations

Referral Instructions

  • Self Referrals or referrals on behalf of a individual can be made by:
  • Once the referral has been submitted the Care Navigation Team will contact the individual

Supporting Information

Shared Decision Making

  • People have a right to make decisions about their care and should be fully informed about the options they face. They should be provided with reliable evidence-based information on the likely benefits and harms of interventions or actions, including any uncertainties and risks, eliciting their preferences and supporting implementation
